With our care at home, patients are assigned to one of our skilled registered nurses who will follow their recovery process from start to finish. By doing this, our registered nurses are able to spend quality time performing assessments to meet the patient’s needs. In a long-term patient-nurse relationship, the nurse knows the patient’s history from first-hand experience. This knowledge allows the nurse to integrate new information to make decisions based on the whole-patient.

Our COPD program provides individualized support and education through clinical pathways to better manage symptoms and improve the quality of life. Those who will benefit may have exacerbations of symptoms, frequent infections and changes in weight. Our coordinated interdisciplinary team can improve quality of life, provide education on oxygen conservation and prevent hospitalization to the emergency room.

Our Congestive Heart Failure (CHF) rehabilitation program assists patients in achieving their optimum quality of life by teaching the skills necessary for self management of their condition. Our program will provide education on signs and symptoms to help with weight changes and diet. Along with education on fall prevention, medications for symptom control and individualized exercise programs. Those who will benefit may have shortness of breath, cardiovascular disease, cardiomyopathy and pitting edema.

The purpose of our continued wellness program is to maintain communication and build relationships with our patients throughout their continuum of care. Even after active services have stopped. At Carter Health care, your health is always important to us. Whether you are actively receiving care or not, we want to make sure you are happy, healthy and mobile.

Our program is personalized to empower our patients with diabetes self-management tools, education and awareness. This is to better manage diabetes, reduce related health problems and improve their quality of life. Carter Healthcare also integrates The American Diabetes Association processes into our program. Our coordinated multidisciplinary team will educate on diet, monitor medication compliance and blood glucose levels. They can also perform venipuncture and educate on the awareness of early warning symptoms. Those who will benefit may have unstable blood sugar, medication and weight changes, along with non-healing wounds.

Our nurses are highly skilled and trained in IV therapy. Our clinicians utilize the latest technology to record vital information during the evaluation of the patient. We have an in-house experienced pharmacy team with over 60 years of experience. Our pharmacy meets the most stringent quality standards (USP 797). Types of IV therapies include antibiotics, anti-inflammatory therapy, anti-viral therapy, pain management and total parenteral nutrition. Our Intravenous Infusion program can prevent hospitalization and emergency room visits.

We know that patients would rather have orthopedic rehabilitation in the comfort of their own homes. Carter Healthcare’s treatment protocols are based on the most progressive national standards, which help support our patients' return to the highest level of independence as possible. Our orthopedic rehabilitation multidisciplinary team can provide therapeutic home exercise and endurance training. Recognition of symptoms and complications of orthopedic disease process. Along with post-op nursing assessment and care. Those who can benefit may have total joint replacement, arthritis, neck and back disorders, traumatic injury or factures.

Our psychiatric program offers comprehensive, Medicare certified psychiatric nursing services to patients in their home. We work with the patient’s primary care physician and psychiatrist, as well as involved community providers. Our psychiatric home care program provides a psycho-educational approach to allow patients to stay in their own homes and communites. Those who will benefit may have serious or persistent mental illness, exacerbation of chronic schizophrenia, or a history of medication non-compliance.

Our pulmonary rehabilitation program assists patients in achieving optimum quality of life by teaching the skills necessary for self-management of their condition. The program’s multidisciplinary team, including nurses, social workers, physical, occupational and respiratory therapists, promotes behavioral skills and techniques tailored for each patient throughout the program. Nursing focuses on education for disease process, diet and medication compliance. Physical Therapy concentrates on building strength and endurance. Occupational Therapy works on building upper-body strength along with muscle training and oxygen conservation. Those who will benefit may have Pulmonary Fibrosis, Cystic Fibrosis or other chronic pulmonary diseases.

Speech-language pathologists play a primary role in the screening, assessment and treatment of cognitive-communication disorders. In addition to cognitive-communication problems, dysphagia is often present. Our speech-language pathology program can address motor speech, swallowing, therapeutic memory techniques and alaryngeal communication to name a few. Those who can benefit may have Dementia, Parkinson’s disease, Huntington’s Disease or suffered traumatic brain injury.

Our multidisciplinary team works together with the patient to customize the patient’s care, maximize the level of functioning and reduce the potential for complications in recovery. The stroke recovery program can increase functional mobility, promote home safety and fall prevention and education on the awareness of early warning symptoms. Those who can benefit from this program are patients dealing with paralysis, weakness, dysphagia and/or a cardiovascular accident.
