Carter Healthcare’s Continuity And Resourcefulness to Empower (CARE) at Home

“CARE at Home enables patients to become as independent as possible within their environment.”

At Carter Healthcare, we believe that continuity of care is consistent with quality, total patient care. With our CARE at Home program, patients are assigned to one of our skilled Registered Nurses who will follow their recovery process from start to finish. Our RNs are able to spend valuable patient time performing focused assessments to meet patient needs, achieving care maximums on each visit. CARE at Home will help gain confidence from our patients while enabling our nurses to be more effective patient advocates.

The close patient-nurse relationship gained from our CARE at Home program gives the case manager a more comprehensive understanding of the patient’s full medical history. In addition to extensive investigation and record review, CARE at Home will also allow our nurses to apply new information and make decisions based on a whole-patient perspective. The patient is provided with consistent, dependable access to one point of contact that can ensure they have direct access to the community resources they need.

Carter Healthcare's Staff

Carter Healthcare hires highly-skilled employee who perform their initial training at our corporate headquarters in Oklahoma City by our experienced education department. Centralized training allows Carter Healthcare to establish a consistent training regiment, outlining the expectations of quality work ethic and standards of care.
